My foreskin is too tight. With some lubrication it can be pulled back but on erection it pains a lot. I tried to stretch it in the above mentioned way, but it has not helped. Is circumcision necessary or can some topical cream etc. help me out?

Dear Dan,

This is not all that uncommon and what is usually recommended is gradually stretching it with gentle manipulation like you described. I have no idea how old you are or how long you have tried stretching your foreskin. Using some topical desensitizing cream for a while might make sense. When I was in rehab with new hip joints, I was given a pain killer before the PT started stretching my joints. However, if none of your home solutions work out, the next step is going to a urologist for more information. But first, go online and contact other men who might have dealt with this same problem. A painful foreskin would be like having a clitoris that's too sensitive to play with. No fun! Or a tight vagina that can't be penetrated. Remember, when we get sexually turned on, pain is diminished. Hopefully after several sessions of masturbation all the way to ejaculation with orgasm your foreskin will relax.

FGM

A paucity of comments/questions. Where is any mention of FGM? 3 million little girls are hacked and butchered every year, many of them sewn up so that they can barely urinate. The mortality rate is upwards of 40%. I'd like to see some comments on this horrific and barbaric practise.

Use the search facility at the top of the page - there are a number of articles about it. Why they don't appear in the 'Circumcision & FGM' category (on the list to the left) I'm not quite sure.

1st I had a Frenularplasty which is a small op to detach the frenula and give me more room. That only eased the situation but didn't  cure it. So I then had a Preputioplasty which is a cosmetic op that leaves your foreskin looking as if nothing has been done to it. That worked, and so far (touch wood) I haven't had any probs with it.

The foreskin is a double layer of skin. The surgeon cuts downwards through both layers and then just sews the layers together. It's quite difficult to explain but once you get it, it's elegantly simple.

There needs to be worldwide outrage about FGM---it ought to be illegal everywhere and banned immediately. I hope the UN and WHO are involved in this effort. Diplomatic pressure might help, too.

I had a woman professor of anthropology at university who really enjoyed making us squirm by telling us about a tribe that practiced male genital mutilation. The penis was split almost end to end like a sausage, which greatlly reduced sexual function and made intercourse all but impossible. As I recall, this was apparently done to reduce the birth rate, since there were limited resources available where they lived, even for a small number of people. But wherever mutilation is taking place, it's a terrible and barbaric thing and must be stopped.
